The following forms are used for the First Round and Second Round PPP Loan applications:
- First Round PPP Loan Application (Revised SBA Form 2483) for other than Schedule C
- First Round PPP Loan Application for Schedule C (SBA Form 2482C)
- Second Round PPP Loan Application (Revised SBA Form 2483-SD) for other than Schedule C
- Second Round PPP Loan Application for Schedule C (SBA Form 2483-SD-C)
- Our Form 100 - Document Checklists (required of both First and Second Round Applications)
- Our Form 101 - Supplemental Borrower Information Form (required of both First and Second Round Applications)
- Our Form 103 - Loan Amount Calculation Sheet (required for (a) First Round Loan Applications and (b) those Second Round Loan Applications that are with a different lender or a different base period (e.g., 2020 vs. 2019)
- Our Form 112 - Consent for Advisor to Access Information (OPTIONAL) - used if you want a third-party to have access to your data.
- Our Form 151a - Bank Deposit Information for transfer of loan proceeds
- Our Form 199 - Statement of Intent and Consent to Use Electronic Signatures - used if you are having problems with the electronic signatures on the SBA forms.

Note: Effective February 1, our lenders are no longer requiring that documentation on the reduction of 25% more more of revenue required for a Second Draw PPP loan be provided, to be consistent with the SBA Second Draw application form. This documentation must be provided on or before forgiveness can be granted. If the reduction cannot be documented, the loan will not be forgiven and must be repaid. Our Form 101 - Supplemental Borrower Information - has been updated to reflect this change and
our Form 110 has be eliminated.
